I, Contractor, hereby terminate my Contract employment with Tricks For Treats Jax, and understand that I am still bound by the Contractor Agreement & Policies Contract and details signed during my time of employment.

Contract Termination Date

Following the termination of contract, the contractor must:

1. Conduct themselves in accordance with Articles A. #10,#11,#12,#13,#14,#15, #16, and #17 of the Contractor Agreement & Policies Contract during and following the termination.

2. Return all provided materials and tools in accordance with Article A. #10 of the Contractor Agreement & Policies Contract.

I agree and understand that these materials provided to me must be shipped to, or returned to Tricks For Treats Jax within two (2) weeks of submitting my termination of contract.

I agree and understand that if any items/materials listed are not shipped/returned and received by Tricks For Treats Jax by the two (2) week period following my termination I will be held responsible and billed for all replacement costs of the listed materials/tools incurred by Tricks For Treats Jax per the Contractor Agreement & Policies Contract Article A. #10.

I agree and understand I have still legally binded by the Contractor Agreement and Policies Contract, and agree to act in accordance with Articles A. #10,#11,#12,#13,#14,#15, #16, and #17 of the Contractor Agreement & Policies Contract during and following the termination of contract.
